HAS READ AND ACCEPTS THE FOLLOWING CONDITIONS:
• That the minor's chaperone, whether their father, mother, legal guardian or an authorised individual, shall be responsible for the minor and shall ensure their well-being and safety, as well as compliance with the rules and conditions, for all purposes, from the moment they enter the venue, during the event and until they exit the venue, in accordance with the obligations and provisions established in the regulations applicable to Catalonia regarding public events, namely Decree 112/2010, of 31 August, enacting the Regulation on Public Shows and Recreational Activities.
• That as the chaperone (father, mother, legal guardian or authorised individual), they are familiar with the content of the aforementioned regulations and, in particular, the specifications concerning minors entering venues intended for adults, where public shows, concerts, etc. provided for in Article 53 and et seq. take place.
• That the chaperone (father, mother, legal guardian or authorised individual) will enter the venue with a maximum of five minors and must present, for this purpose, a QR code for each minor they are accompanying.
• That the chaperone (father, mother, legal guardian or authorised individual) and the minor may be asked by members of the organisation's staff to prove their identity.
A breach of any of the above conditions may lead to expulsion from the venue of both the person signing this authorisation and the individuals for whom they are responsible.
